eXtend Homeschool Tutorial

Family Registration Form:

1. Enter the Parent Name at the very beginning of the registration form where it says “Student Information”
2. Enter the student name(s) in the “Additional Information” section

Each family must be registered with eXtend Homeschool Tutorial to participate in classes. Registration fees are as follows:
$60 - first student
$60 - second student
$60 - third student
$60 - fourth and any subsequent students

**When completing registration form:

1. The PARENT NAME goes in the first field that says “Student Name”
2. Student Names are listed in the additional form fields as indicated on the form
